I bought this 1.) to supplement him at the breast and 2.) to possibly trick him into staying latched without the shield by giving him an instant "reward" of milk. At first I slipped the tube under the shield. Then one random sleepy evening he didn't scream when flesh instead of silicone touched his lips, so I got him to latch - slowly, badly, as he had never latched without a shield before. But it worked! My husband gave the SNS bottle a few gentle squeezes at the start of the feed to entice our son into staying latched,and it worked! We still have issues, but at least now he will try to latch without the shield (with or without the SNS) because he knows he can get milk that way.
It is tricky to put together but worth it compared to the other non-Medela SNS on the market. It WILL NOT leak if assembled correctly. You must make sure that the bumps in the silicone disc that holds the 2 tubes are slipped into the grooves of the plastic disc perfectly - which is NOT hard but is easy to overlook. If the bumps do not align, it will break the seal and it will leak from the cap when overturned. I have always used the slowest flow tubes..the others are pretty fast. If supplementing with breastmilk, be sure to gently heat it to body temp because any bits of solid fat (from being refrigerated) can clog the tube. If you must use powdered formula, mix it at least an hour before using the SNS and warm it to prevent blobs of powder clogging the tube. Ready-made formulas work better.
I have not had trouble with the tubes staying in the clamps, which are the grooves all around the cap's ring. After testing the SNS before putting it on (to make sure the tubes are not blocked), I tuck the tubes in and have no leakage issues.
To prevent the tubes from leaking while nursing (usually, the tube on the opposite breast needs to be open so that it can vent while baby takes milk from the other side) I tape it up to both sides as suggested, and only unclamp the side I mean to use right before trying to latch. After he is latched for about 30-60 seconds, I will slip the other side out of the clamp. This way there is already a vaccum and instead of the other tube leaking, air will flow in and vent the bottle.
It is tricky to get Baby to latch with the tubes, but it can be done. Place the provided tape as suggested, and place an additional piece of tape ON the areola, very close to the nipple, so that the tube is right there and cannot be flipped away. It did not affect my son's latch. The tape is hard to remove in this sensitive spot, but just use a little lanolin, baby oil, or even cooking oil to soften the adhesive/
I wish I'd bought this sooner! My supply went up, Baby's coordination got better, his nipple preference/confusion went down, and we were all around happier. I'd definitely buy it if you have issues, or buy Medela Starter Supplemental Nursing System (SNS) Sterile M#00902 without 80ml Bottle , the temporary, cheaper SNS before Baby arrives if you are worried about having troubles
On the surface it seems brilliant. Increase the supply of milk during breastfeeding to teach your newborn that the nipple is a reliable supply of food.
Problem one: engineering. The makers of this product do not understand pressure. There is no hole in the "bottom" of the container. This means that as the newborn sucks milk out of the tube, no air is able to balance the pressure in the container. Almost immediately the pressure inside of the container is greater than the pressure outside. The result is the flow of milk stops. This means the only way to continue the flow of milk is to squeeze the container pretty hard. If you're having trouble with your newborn during breastfeeding, the last thing you need is another activity for your hands.
Problem two: practicality. the two tubes are just asking to get tangled in your newborn's hands.
Problem three: maintenance. Every single part is a hassle to clean. They cannot be cleaned in the dishwasher and they can't even be cleaned with your other hand washed parts. The bottle is tied to the rope, so you either need to detach and reattach the rope every cleaning, or make sure to keep it dry. The tubes are incredibly thin and require special attention. They cannot be dried so you basically have to disinfect after every use.
Summary: We are all here for the same reason. We hope this will be the key to successfully implementing a maintainable breastfeeding cycle. Despite my review, many of you will probably still purchase this tool as you figure it is your last chance. Many a first time parent like myself is willing to do whatever it takes for their little one. I'm sorry to say not only will this not solve your problems, but will compound them.

The price is outrageous for the actual materials, but the design and the fact that it allowed my child to have the best of both worlds means it is well worth a purchase.
Pros:
- when well settled, the baby sucks as there is nothing else but the breast. It's very indiscreet.
- no need to use bottle (without nipple confusing issue)
- you can put all kinds of liquid inside like the bottle, but all the baby's sucking effort is used to stimulate your breast (instead of a piece of silicon on the bottle!)
- physical and natural stimulation of breast (not like medication, you never know the effect), and the baby sucks better than a pump!
Cons:
- it takes several days of practice to master how and to know when to discreetly insert the tube into the sucking baby's mouth.
- you need someone to assist you in the beginning, until you master the inserting technique. (I always had another person helping me because I always had two babies crying for milk... but I believe mum of singleton can easily handle it)
- it's a bit more difficult to clean than a bottle.
When your goal is to boost breast milk production, using SNS is the best way! I cannot understand why SNS is not as strongly advertised as the other nursing systems (like Dr Brown's bottle), at least in Canada. Personally, I used 90% of the time SNS and 10% bottle (during the night it's faster and easier with bottle). I used SNS for 3-4 months, until I was back to work. SNS is the best lactation product for a mother wishing to increase or keep breast milk production. And even if you believe your will have enough milk, it's a useful tool for the first days after birth to have a good start for nursing.
